Understanding Breast Implants

Breast implants come in a variety of options. All implants have a durable silicone outer shell and are filled with either medical-grade silicone gel or saline (sterile saltwater). At The First Glance, we exclusively use smooth, round breast implants, which come in sizes ranging from 100cc to 800cc.

The profile and projection of the implant play a significant role in achieving the desired breast shape and upper fullness. Dr. Islur will discuss these details with you and recommend the best options to help you achieve your desired look.

Breast Augmentation with Implants

When it comes to breast implants, there are a wide range of options available.  All breast implants are made of a highly durable silicone shell and can either be filled with medical grade silicone or saline (sterile salt water).  At present, we only use smooth round breast implants.  Volumes for these implants can range from 100cc/grams to 800 cc/grams.

Desired breast shape and the amount of upper breast fullness can be obtained by the Profile/projection of the implant. Dr. Avi Islur  will discuss your options and suggest the best ones to suit your desired outcome.

Breast Augmentation with Saline Filled Implants

Saline-filled implants provide a different option from silicone gel-filled implants. Made with a strong silicone outer shell, these implants are filled with sterile saline (saltwater) in the operating room through a one-way valve. They offer flexibility in volume, allowing for a wide range of sizes, and are available in various profiles to suit different aesthetic preferences.

Breast Augmentation with Silicone Gel-Filled Implants

Silicone gel-filled implants, often known as “gummy bear” implants, are made with a form-stable cohesive gel that retains its shape and won’t deflate. The latest generation of these implants has a very low risk of leakage or rupture and comes pre-filled and sterile from the manufacturer. You can choose from different levels of gel cohesivity, ranging from soft to firm, to achieve the desired feel and to maintain shape in the upper part of the breast when standing. These implants offer a wide variety of volumes and the most extensive range of profiles available. Silicone is considered very safe and is the gold standard for biological compatibility within the human body.

Breast Implant Incision And Position

Implants can be inserted using two types of incisions: through the breast crease (inframammary incision) or around the edge of the nipple (peri-areolar incision). The inframammary incision, made in the breast crease, is the most common and safest approach because it avoids scarring breast tissue and minimizes the risk of contamination from breast cysts. Additionally, this method helps maintain nipple sensation. To keep incision lines small (between 3-5 cm, depending on implant size), Dr. Avi Islur uses a Funnel technique that reduces the incision size by 40 to 60% compared to traditional methods.

Implants can be placed in one of two anatomical positions: above or below the pectoralis major muscle.

  1. Sub-glandular/Sub-fascial Placement (Over the Muscle): This placement involves positioning the implant above the muscle, beneath the breast tissue or fascia. It is often chosen for patients who have a moderate amount of natural breast tissue and want to maintain maximum muscle strength.

2. Dual-Plane/Sub-muscular Placement (Under the Muscle): This is the most common placement for breast augmentation. It involves positioning the implant partially under the chest muscle. The dual-plane sub-muscular placement has the lowest risk of long-term complications, such as capsular contracture, rippling, and implant visibility. It also provides optimal breast tissue visibility for mammographic screenings and creates a more natural breast appearance, even with larger implants. However, there may be some loss of muscle strength, and it is advised to avoid frequent chest muscle workouts after surgery to prevent “animation deformity,” where the implant moves noticeably with muscle contractions.

How Much Do Breast Implants Cost?

Length of Breast Augmentation Surgery

Breast augmentation with implants is performed under general anesthesia in about 1 hour. If combined with a breast lift, the time of surgery can range from 1.5 hours to 2.5 hours.

Recovery Time Breast Augmentation with Implants

After breast implant surgery, you can start moving around the same day once the anesthesia wears off. Dr. Avi Islur will prescribe pain medication to manage discomfort and antibiotics to prevent infection. You may experience swelling and mild bruising, but you will notice an immediate change in your breast size. However, keep in mind that the swelling needs to subside before your final size is apparent. You’ll be given a compression bra to wear after surgery to help with swelling and support.

Recovery Timeline for Breast Augmentation with Implants:

  • First Week: Bruising and swelling will peak within the first 5 to 10 days and then start to decrease. You can shower the day after surgery and return to non-strenuous work in about a week.
  • After 2 Weeks: Light to moderate exercise and full arm movements can typically be resumed. Most of the swelling will be significantly reduced, and the final shape of your breasts will begin to emerge.
  • Full Recovery: Complete recovery may take up to 8 weeks, but many patients feel fine after 1-2 weeks.

Important Tips for Breast Augmentation Recovery:

  • Time Off Work: Plan to take 4 to 7 days off work. Recovery time varies; some may need more or less time depending on their job’s physical demands. Dr. Islur will provide a personalized recovery plan during your consultation.
  • Confidentiality: Doctor’s notes for time off work are available and will not disclose the type of surgery.
  • Exercise Restrictions: Avoid strenuous aerobic activities, heavy lifting, and exercises that increase heart rate or cause significant breast movement for 2 to 6 weeks. Dr. Islur will provide specific guidelines based on your surgery.
  • Sleep Position: Do not sleep on your chest for 8 weeks. There is no need for breast massage after surgery.
  • Custom Care: Your recovery instructions will be tailored to your specific needs, and results may vary. If you are an out-of-province patient, follow-up instructions will be provided during your consultation.

While everyone heals differently, most patients feel ready to resume normal activities within a couple of weeks. Always remember that your recovery is unique to you.

Frequently Asked Questions: Breast Augmentation with Implants

Despite its popularity in the past, this is a practice that should remain in the past. Unless otherwise directed by Dr. Islur, massaging your breast implants should not need to be performed. The way you sleep, exercise, and move around daily will move your implants enough that adding external massage should not be performed. Unfortunately, if you massage your breasts implants too early in the healing phase, the precise “hand-in-glove” pocket that was created for you can become loose and big which can result in implant malposition.

If your implants have been placed over the muscle, you are free to work out on your chest (pectoralis major) muscle as much as you desire; however, if your implants have been placed under the muscle, it is generally best to avoid strenuous isolated chest days or multiple chest exercises at the gym. It is ok to do 30 push-ups a week or include bench press in your workouts once per week but overdoing isolated chest days can result in the pectoralis major becoming too strong which will result in wider cleavage and potential lateral and downward displacement of the implant. Remember, almost all upper body exercises (biceps, triceps, shoulders etc.) will engage the pec muscle as an accessory stabilizing muscle anyway – these exercises are ok to do.

In the past, when using implant sizers in a bra typically the results were about 10% smaller than expected; however, with our 3D simulation imaging software, the results you obtain after surgery are very close to your simulated pictures. So if you like what you see on the simulation, stick with those!

Absolutely! It is very important to get breast mammograms and perform breast screening as breast cancer occurs in 1:8 woman during their lifetime. Despite the compression used in mammograms, breast implants are very durable and will not rupture. Make sure you seek out an appropriate facility that has the technology and expertise to perform special views (Eklund views/distraction views) required to see the breast tissue which lies on top of the implant. As a secondary benefit to the mammogram, your implants will also be visualized to detect any issues with them. An MRI can also be performed if there are concerns with having a mammogram.

For changes of this nature, fat grafting is likely the best option; however, you must have an adequate amount of fat on the abdomen or flanks to harvest (liposuction). Small breast implants can be chosen as well, but generally provide more than a 1 cup size increase.

Breast implants are not a lifetime device. At some point in your lifetime, your implants will likely need replacement or removal depending on your wishes but they can definitely last you longer than 10-12 years. All breast implants come with a lifetime warranty against leakage and rupture. The ability to detect if they have ruptured or leaked depends on the type of implant fill – silicone or saline. With saline implants, the breast size will change immediately if a leakage or rupture has occurred and this will be easily detected. With regards to silicone gel implants, the cohesive nature of the silicone gel is meant to not change shape even with a leakage or rupture hence the name “gummy-bear”. With these implants it is generally recommended that a breast ultrasound be performed 3-5 years after implant placement to look at the integrity of the implant. I have always had a smaller chest and was always thinking of get a breast augmentation for a few years now and I finally had the courage to go through with it and I’ve got to say it was the best decision of my life. I am now 1 month post op and I couldn’t be happier with my results. A lot of people are surprised at how natural they look and that was my number one concern as I was afraid they would look fake on me. This was a decision I made for myself and no one else and I don’t regret it for one second. I’ve always wanted larger breasts (5’9″ / 150 lbs / B cup at VS) and finally decided I deserved it. The biggest piece of advice I have is to go for a consultation and learn more about it. This really helped me understand the whole procedure and what was involved and made me immediately comfortable. If I felt uneasy about things during the consult, I would have changed my mind or gone elsewhere. Find a place you trust. Mine were done over the muscle (don’t believe the rumours – over/under isn’t better/worse, it’s about body type and the shape you’re going for) and I had a relatively easy recovery. Surgery on Monday afternoon and I was back at work Thursday at my office job, though I probably should have waited until Friday.
The best question I was asked during my consult was: “what would you be more upset about – waking up and seeing that they were a bit smaller than you wanted, or waking up and seeing that they were a bit larger than you wanted?” This made all the difference because you really learn what you’re looking for.
